SIP phone can no longer register

Hello, I've had an Actiontec M1424WR for just over a year and have been using a Polycom SIP phone to register to my office so that I can work from home.  Sometime in June I can no longer use my phone.  It attempts to register, I see the registration on my office server, as well as their reply.  Unfortunately I never see the reply hit my home network.

 

I've played with port forwarding as well as other configs in the router, but no luck.  This had been working just fine without any forwarding configs prior to June.

 

I see similar issues here, but I have yet to see one resolved.  Verizon tech support has not been able to pin this down either.

 

I beg you Obi-Wan, you're my only help.
