Need self return box and label for Actiontec router

Hello, my Fios disconnect date was May 13, 2015. I haven't yet received the return box and label. Can you send another box and label or have I just not waited long enough to receive it? (I was told I needed to return the Actiontec mi424wr router). Thank you.
